Sparta Middle School Survey

Hi All,
I'm looking for some ideas for a survey we would like to put out to the Sparta Middle School staff. We would like to offer opportunities for our staff to improve their physical and emotional well being. Our interest survey would guide us to provide events that the staff is interested in. Some ideas that have been offered are: hiking, biking groups, rafting trips, walking club, nutitionist consultations, massage therapy, relaxation therapy. Do you have any suggestions? What would you like to see offered? Thanks for your help.
